Cardiovascular Patient Navigator

The Cardiovascular Patient Navigator is an advocate for patients in need of health care services. This registered nurse provides support and guidance to help patients "navigate" through the complex health care network. This is a free service offered courtesy of St. Peter's Hospital.

Our Patient Navigator team has more than 20 years of healthcare experience and is dedicated to providing more effective and efficient health services to patients. These navigators are committed to assisting you throughout your hospital experience. Each team member maintains flexible hours and direct communication with their patients. 

What Does a Navigator Do?

From assisting with referrals to connecting patients with financial assistance, the patient navigator provides patients and their families with a variety of helpful services. The navigator offers patients and families:

  • Seamless and timely coordination of care
  • A consistent contact person throughout the care process
  • Improved access to community resources and support services
  • A better understanding of disease, treatment options and care plans 

Connecting Patients to Information and Services

The navigator offers an individualized approach to help patients better understand cardiac care from diagnosis to treatment. Working directly with the patient, the navigator will assess the patient's clinical, emotional, spiritual, financial and other needs or concerns.

The patient navigator helps individuals to overcome issues with finance, language, literacy, discharge, transportation, and other challenges.

It is the navigator's goal to support the patient's active participation in their own health care, which may include the following areas:
