Discharge Planning & Support
St. Peter's offers counseling and support to patients during and after their stay with us. The services listed below are part of the discharge planning process administered by the Case Management Department.
A case manager will help you plan your discharge needs prior to your leaving the hospital. This may include coordination of home care, CHOICES or hospice; delivery of medical equipment; or transfer to a rehabilitation center, skilled nursing or assisted living facility.
CHOICES is an elder care consulting program to help adults aged sixty and older maintain their independence through the use of comprehensive services available in their community.
Behavioral Health - Counseling & Support
Patients suffering stress, anxiety and other mental-health issues can consult with St. Peter's Behavioral Health service.
Palliative Care - Coping with Serious Illness
For patients facing serious, life-altering illnesses, the Palliative Care team provides emotional and spiritual support.
