Inpatient Social Work
A staff of master's trained social workers is available to assist patients and their families in coping with the physical, emotional and financial challenges which may accompany an illness. Social work services include:
- counseling—providing short-term counseling and goal-oriented interventions that assist patients and families with life transitions such as illness, injury, disability and loss.
- advocacy—advocating for patient rights, including the right to make informed decisions about one's health care and the right to patient dignity and self-determination.
- community resource and long term-planning—working with patients, families, physicians, nurses and other health professionals to plan post hospital care. This includes assessing patient needs and recommending referrals to appropriate community resources.
Our registered nurse case managers are very knowledgeable about home care and rehabilitation resources, as well as issues regarding your insurance and what kind of services may be covered upon discharge from the hospital.
