Community Health

St. Peter’s Hospital, like all of St. Peter’s Health Partners, is committed to improving the health and wellbeing of our community, not only as a caring community member, but also as a catalyst for change.

As such, we participate in many community partnerships aimed at assessing the current health status of our community and identifying opportunities to make a difference in the health of our citizens with particular attention to those who are poor and vulnerable.

As we have done for many years, we continue to play a major role in the Healthy Capital District Initiative, an organization dedicated to improving the health of the residents of Albany, Rensselaer and Schenectady Counties. Our partners in this endeavor are the local county health departments, other health care providers, insurers and community members.

In June 2016, this group completed the 2016-18 Community Health Needs Assessment (CHNA) , which represents a current profile of health status, wellness, health delivery and public-sourced opinions about health in the Capital Region and identifies significant health issues in the communities we serve. In October 2016, the Community Health Improvement Plan was finalized.

This assessment contains both quantitative and qualitative data sources, along with significant stakeholder and public input. This information will be used in the development of a Community Health Improvement Plan (CHIP) that will guide St. Peter's Hospital, as well as St. Peter’s Health Partners Community Benefit programming and activities, for the next three years. It will also help us track our progress.

The CHNA and CHIP together form St. Peter’s Hospital's Community Service Plan. This document should not be viewed as a static document but as a dynamic roadmap that will improve the health and well-being of residents in the Capital Region.
